INC. and _____________________________________________________, the owner (s) of the dog (s)

Please initial next to each item to certify that you have read and agree with each statement.
1. _____ I understand that I am solely responsible for any actions or harm caused by dog(s) while my dog
(s) is/are attending Alexander’s Doggie Daycare, including but not limited to any medical and/or legal bills.
2. _____ I understand and agree that in admitting my dog(s) to Alexander’s Doggie Daycare, their staff has
relied on my representation that my dog(s) is/are in good health and has/have not harmed or shown aggressive
or threatening behavior towards any person or any other dog.
3. _____ I understand and agree that by admitting my dog(s) to Alexander’s Doggie Daycare for daycare
and any other services that they provide, there are certain risks inherent with a cage free environment that
include but are not limited to injury and illness from interactions with other dogs and that Alexander’s Doggie
Daycare, their staff as well as any volunteers and affiliates and Watland Inc. will not be liable for any injuries or
harm to my dog(s).
4. _____ I understand and agree that Alexander’s Doggie Daycare, their staff as well as volunteers and
affiliates and Watland Inc. will not be liable for any injuries or damages t my dog(s) arising out of or from the care
of my dog(s) including loss by fire, theft, injury or death while under the care of Alexander’s Doggie Daycare
provided that reasonable care and precautions are followed and I hereby release them of any liability of any kind
whatsoever arising from my dog’s attendance at Alexander’s Doggie Daycare.
5. _____ I understand and agree that any problems that develop with my dog(s) will be treated as deemed
best by the staff of Alexander’s Doggie Daycare at their sole discretion and that I will assume full financial
responsibility for any and all expenses involved.
6. _____ I agree to indemnify, defend, and hold harmless Alexander’s Doggie Daycare, it’s affiliates,
volunteers and staff and Watland, Inc. from injury or harm of any kind whatsoever to third parties.
7. _____ I understand and agree that Alexander’s Doggie Daycare reserves the right to change prices,
fees and hours of operation without notice.
8. _____ I understand that payment in full is due when services are rendered unless arrangement were
made with management prior to the date services were provided.
